WIN/LOSS STATEMENTS

If you would like to receive a win/loss statement of your gambling winnings, please visit the Main Players Club 24 hours a day and your statement will be provided to you. You may also contact us by email at winloss@valleyviewcasino.com or by phone at 760-291-5507 with the following information and we will mail or fax your request:

  • First and Last Name
  • Players Card Number
  • Date of Birth
  • Current Address or Fax Number
  • Which year’s win/loss statement you are requesting
  • Preferred method of delivery; pick up at the Main Players Club, fax or mail

All voice-mail requests are processed in 72 hours or less. If choosing to receive your win/loss statement by mail, please allow up to two weeks for delivery. Thank you.


DUPLICATE TAX FORMS

If you would like to receive a duplicate W2G for gambling winnings or duplicate 1099Misc for promotions or giveaways, please contact our automatic voice-mail message line at 760-291-2333 and leave the following information:

  • First and Last Name
  • Players Card Number
  • Date of Birth
  • Last 4 digits of your social security number
  • Current Address or Fax Number
  • Which tax year and tax form you are requesting
  • Preferred method of delivery; pick up at the Main Players Club, fax or mail

All voice-mail requests are processed in 72 hours or less. If choosing to receive your duplicate tax form by mail, please allow up to two weeks for delivery. Thank you.
