If you would like to receive a win/loss statement of your gambling winnings, please visit the Main Players Club 24 hours a day and your statement will be provided to you. You may also contact us by email at email@example.com or by phone at 760-291-5507 with the following information and we will mail or fax your request:
- First and Last Name
- Players Card Number
- Date of Birth
- Current Address or Fax Number
- Which year’s win/loss statement you are requesting
- Preferred method of delivery; pick up at the Main Players Club, fax or mail
All voice-mail requests are processed in 72 hours or less. If choosing to receive your win/loss statement by mail, please allow up to two weeks for delivery. Thank you.
